My Healthy Health Plan

My Healthy Health Plan logo

My Healthy Health Plan is a web-based program with tools to help you meet your health and wellness goals. It includes a health assessment that you can complete to get an overall picture of your health. It also features an online library with articles, quizzes, how-to guides and recipes.

Logging into My Healthy Health Plan

  • Access to My Healthy Health Plan is though Fallon Health's member portal, MyFallon.

  • If you have a username and password that you use to get into Fallon Health's Document Center or Preference Portal, you'll use that same username and password to log into MyFallon

  • If you don’t have a username and password to that you use to get into Fallon Health's Document Center or Preference Portal, you can register to use MyFallon, and then log into MyFallon.

  • After you log into MyFallon, go to the "My Resources" link at the top of the screen, and then choose "Healthy Health Plan" from the menu that appears.

More about the health assessment

The Health Assessment is a health and lifestyle questionnaire that you can complete to get an overall picture of your current health.

Once you finish the assessment, you'll get an individualized report with your overall score, areas in which you’re doing well and areas in which you need improvement. If you're already in great health, terrific! If you could use a little help to get healthier, you'll have access to health coaching and an online wellness library that includes articles, quizzes, self-assessments, how-to guides and healthy recipes.

If you join now, or have already been a participant, we'd like to get your feedback to help us evaluate and improve our program. Please take a couple of minutes to complete this short survey

At this time, The Healthy Health Plan is not available to our NaviCare, Fallon Medicare Plus or, effective April 1, 2020, to our MassHealth members. Also, if you're part of a large employer group, your employer may have other wellness programs in place and may have chosen not to participate in this one. Your Fallon Health Schedule of Benefits document, (which you can find by logging into the  "Document Center" area of MyFallon) will tell you if you have the program.

When can I start using
My Healthy Health Plan?

My Healthy Health Plan is available to you upon your health plan anniversary date. For example, if your plan renews on March 1, you're eligible to take advantage of the My Healthy Health Plan as of March 1.

Some plans may not have access to My Healthy Health Plan. Your Fallon Health Schedule of Benefits document (which you can find by logging into MyFallon) will tell you if My Healthy Health Plan is available to you.

Incentives

Members of some plans (Community Care, Direct Care, Select Care and Preferred Care) may be eligible for incentives when they participate in the Healthy Health Plan. For more information, see the Healthy Health Plan flyer (pdf).
