Pharmacy finder look-up
With this pharmacy finder look-up tool, you can look up more than 64,000 pharmacies that are included in our pharmacy network. FCHP contracts with pharmacies that equals or exceeds the Centers for Medicare & Medicaid Services requirements for pharmacy access in your area.
If you get your health insurance through your or your spouse’s current or former employer, contact the benefits administrator. Your pharmacy network may be different than the pharmacies than are listed here.

To find a pharmacy:
- Click on “View the entire list” to view all of the pharmacies in our network*
or
- From the drop down menu, select the state in which you would like to look up a pharmacy. Then, click on the letter that corresponds to the first letter of the name of a pharmacy’s name.

**The pharmacy network may change at any time. If you need assistance locating a pharmacy or are looking for a particular pharmacy and do not find it, prospective members may call Fallon Senior Plan at 1-888-340-5504 and current members may call 1-800-325-5669 (TTY users, please call TRS Relay 711), Monday through Friday from 8 a.m. to 8 p.m. (From October 15 through February 14, we're available seven days a week.)
