Fallon Health announces Leanna Moran as Site Director of Summit ElderCare Lowell

Lowell, Mass., March 31, 2016— Fallon Health, a nationally recognized, not-for-profit health care services organization, is pleased to announce the recent appointment of Leanna Moran as Site Director of its new Summit ElderCare® location on the campus of D’Youville Life & Wellness Community.

Since joining Fallon in 2011, Moran has held various leadership roles in senior care services. As site director of Summit ElderCare, Moran will oversee the daily operations of Fallon’s Program of All-inclusive Care for the Elderly (PACE) while interacting with participants and caregivers. She will lead an interdisciplinary team that includes a range of professionals who specialize in geriatric care, including primary care physicians, nurses, nutritionists, transportation coordinators, home care coordinators, social workers, rehab therapists, ensuring that each participant’s personalized care plan allows him or her to live as independently as possible.

Prior to this role, Moran was Executive Director of NaviCare® Fallon Health’s Medicare Advantage Special Needs Plan and a Senior Care Options (SCO) program for adults age 65 and older. Moran led the program’s overall operations, growth and continued expansion. She also served as Director of Outreach for NaviCare and Summit ElderCare.

Previously, Moran worked as Corporate Marketing Director for Senior Living Residences, a Massachusetts assisted living provider with a specialty in Alzheimer’s care. She also owned and operated her own guardianship and care management business, serving hundreds of elders throughout Massachusetts.
Moran has a master’s degree in clinical social work from Boston College and a bachelor’s degree in psychology from Brandeis University. She is a member of the board of directors for the Lowell Plan and Roudenbush Community Center in Westford. She lives in Westford with her family.

Fallon’s Summit ElderCare program in Lowell is the company’s sixth location in Massachusetts with other sites in Charlton, Leominster, Springfield and Worcester (two). Fallon also recently opened a PACE program, Fallon Health Weinberg-PACE, in Buffalo, New York. Fallon Health is a leading PACE provider in the country and the largest in New England.
