Fallon Community Health Plan now accepting applications for community grant program

Announcing an expanded funding area across the Commonwealth and new focus areas to include behavioral and mental health issues and at-risk/proven-risk youth and young adults

Worcester, Mass., May 20, 2013 ―Fallon Community Health Plan (FCHP), a nationally recognized, not-for-profit health care services organization, today announced that it will award more than $100,000 in grants to support innovative, community-based programs aimed at one or more of the following priorities:

  1. Promoting opportunities for at-risk/proven risk youth and young adults between the ages of 12 to 21 with a specific emphasis on youth preparing to age-out of the system and lose services
  2. Providing health and social services for seniors
  3. Improving access to good nutrition and promoting physical activity
  4. Providing services to support individuals with behavioral and/or mental health issues with a specific emphasis on addressing the needs of the most vulnerable groups by their ethnicity, age, demographics and risk-factors.

Funding priorities were determined by the U.S. Department of Health and Services’ Leading Health Indicators as well as FCHP’s annual needs assessment which identified the health-related issues and gathered the statistical data to support the findings across the region. Organizations meeting the criteria are encouraged to apply for the grants.

Grants will be awarded to support new, innovative programs or to strengthen existing programs that serve the targeted populations across our entire service area throughout Massachusetts with a particular focus in Barnstable County, Berkshire Country, Bristol County, Essex County, Hampden County, Hampshire County, Franklin County, Middlesex County, Norfolk County, Plymouth County and Worcester counties.
