Fallon Community Health Plan expands NaviCare® HMO SNP service area

Now open for enrollment to eligible seniors in Hampden and Worcester counties and portions of Franklin, Hampshire, Middlesex and Norfolk counties

Worcester, Mass., October 11, 2011 ― Fallon Community Health Plan (FCHP), a nationally-recognized, not-for-profit health care services organization, announced today the expansion of its Medicare Advantage Special Needs Plan, NaviCare® HMO SNP and NaviCare SCO, a Senior Care Options program. In addition to still being offered in Worcester County, NaviCare will also be available (for a January 1, 2012, effective date) to eligible seniors in Hampden County and portions of Franklin, Hampshire, Middlesex and Norfolk counties.

NaviCare is a unique product for seniors that combines in one package all Medicare and MassHealth Standard benefits, services and items, as well as needed community-based home and personal care services such as adult day health, home-delivered meals or help with bathing and dressing needs. NaviCare is available to those residing in the service area that are age 65 and over who are enrolled in MassHealth Standard. For those who qualify, NaviCare is available at a $0 premium with $0 copayments. Members receive high quality care through the program’s ability to combine health and social services. All health care is coordinated by a team of doctors, specialists, nurses, and social workers who work with each member to develop a plan of care specific to them.

Unique to FCHP’s NaviCare program is the role of the “Navigator,” who helps manage the members’ health care needs, including some of the tasks that may be typically performed by a caregiver. For example, the Navigator will help schedule appointments, coordinate pre-approved transportation for medical and rehabilitation visits and make arrangements for medical care and services.

In addition to NaviCare, FCHP also will continue offering other products and programs for Medicare beneficiaries, including a variety of plan choices for its Medicare Advantage product, called Fallon Senior Plan. FCHP also continues to be the only health plan to sponsor one of six Programs of All-inclusive Care for the Elderly in Massachusetts. The program is called Summit ElderCare® and has locations in Charlton, Worcester and Leominster. Summit ElderCare is an insurance, medical care and social support system all in one convenient package that includes home care, personal care and even prescription drugs, when approved.
