As a new member, you will receive:

  • a new member welcome kit with a Schedule of Benefits in the mail
  • a separate mailing with your ID card 
  • a phone call from us to personally welcome you and help answer any questions you might have.

What to do next

We will help you continue your care

If you or a family member are receiving ongoing care for medical conditions or pregnancy, call Customer Service. We'll help you get the care you need with your new FCHP coverage. Call us if:

  • You are pregnant.
  • You are scheduled for surgery or for a CT scan, PET scan or MRI.
  • You use durable medical equipment (e.g., wheelchairs, oxygen equipment), prosthetics or orthotics, or home health services.
  • You receive physical or occupational therapy.
  • You receive ongoing treatment for complex medical conditions, like cancer, diabetes, heart disease or other serious illnesses.
