Who is eligible for NaviCare HMO SNP?

You're eligible for NaviCare HMO SNP if you:

• Are 65 or older.
• Have Medicare Parts A and B,
and MassHealth Standard.
• Live in our service area.

Eligible beneficiaries may enroll in NaviCare at any time.

Who is eligible for NaviCare SCO?

You're eligible for NaviCare SCO if you:

• Are 65 or older.
• Have MassHealth Standard
(and may have Medicare Part A or B).
• Live in our service area.

Eligible beneficiaries may enroll in NaviCare at any time.
