Monday through Friday, 8 a.m. to 8 p.m.
(From October 15 through February 14, we’re available seven days a week.)

If you prefer, you can mail or fax us a completed application at your convenience.

Medicare beneficiaries may enroll in Fallon Senior Plan HMO through the CMS Medicare Online Enrollment Center located at www.medicare.gov.
