MassHealth is changing.

Beginning March 1, 2018, MassHealth members will have more options for getting care, including a MassHealth Accountable Care Organization (ACO) Partnership Plan.

What is an ACO?

An ACO is made up of a group of health care providers who work closely with patients and their families for all of their health care needs.

Doctors and other health care providers choose to participate in an ACO because they are committed to providing patients with a better care experience. In an ACO, your PCP is responsible for coordinating your care and connecting you with available services and supports. This coordination will help you get the right care at the right time, in the right place.

Which ACO Partnership Plans does Fallon participate in?

Fallon has partnered with hospitals, health care providers and community health centers across the state to create three new ACO Partnership Plans to better serve MassHealth patients:

Berkshire Fallon Health Collaborative—a partnership with Berkshire Health Systems, Community Health Programs and several Berkshire County community physician practices. Learn more at fallonhealth.org/Berkshires.

Wellforce Care Plan—a partnership with Wellforce, which is made up of physicians and other health care providers from, or affiliated with: Circle Health; Floating Hospital for Children at Tufts Medical Center; Hallmark Health; Lawrence Memorial Hospital of Medford; Lowell Community Health Center; Lowell General Hospital; Lowell General Physician Hospital Organization; Melrose-Wakefield Hospital; New England Quality Care Alliance (NEQCA); and Tufts Medical Center
