Information for providers

Summit ElderCare can help aging adults with complex health needs stay healthy and at home as long as possible.

When determining if someone is a candidate for Summit ElderCare, there are a range of factors that are considered. The following list includes, but is not limited to, some of the key factors that may be used to determine program eligibility:

  • Wants to avoid living in a nursing home
  • Needs help with daily activities, such as bathing, dressing, meal preparation and light housekeeping
  • Uses insulin or oxygen
  • Has skilled needs, including nursing, physical therapy or occupational therapy
  • Can’t afford copays
  • Needs a wheelchair, walker or other type of medical equipment

Caring for a loved one?

Summit ElderCare PACE supports families and caregivers so that their older loved ones can continue to live at home instead of in a nursing home:

  • Individualized consultation on care plans
  • A Care Team that works closely with caregivers
  • 24-hour telephone access for emergencies and unexpected situations
  • Caregiver support groups
  • Respite

Monthly caregiver support group meetings

Hosted at each Summit ElderCare PACE Center, these meetings are free and open to the public. They feature monthly topics and speakers.
