Summit ElderCare PACE Centers

Summit ElderCare participants have access to one of our PACE Centers—a centralized location where you receive medical care, social services and care coordination in an adult day health setting.

At our PACE Centers, you get:

  • Rehabilitation and speech therapy
  • Nutrition counseling
  • Vision, hearing and dental care
  • Meals and social activities
  • Programming and dedicated areas for participants with Alzheimer’s disease, dementia and other cognitive impairments

How often you attend the PACE Center depends on your care plan. Many participants benefit from regular attendance—which allows the Care Team to address minor health issues before they become challenging.

Social activities

When you join Summit, you become part of the PACE Center community. In addition to providing medical care, our Summit ElderCare PACE Centers offer a way for you to:

  • Make new friends—we offer a coffee social every day
  • Enjoy recreational activities, including games, music, crafts, entertainment and group discussions
  • Have lunch and snacks
  • Arrange for on-site haircuts and hairdressing services

Transportation to and from the PACE Centers is available on an as-needed basis, as long as it is approved by your Care Team.
