Fallon Health Honors Reliant Medical Group with its Moving Forward Award
Care Team model addresses community health care needs
Worcester, Mass., June. 20, 2019 — Fallon Health, a nationally recognized not-for-profit health care services organization, today announced its first annual Moving Forward Award has been presented to Reliant Medical Group.
Reliant Medical Group was selected for the honor based on its “Care Team Clinical Model,” an innovative approach in which various caregivers work as a team to care for groups of patients. Physician-led teams comprising advanced practitioners, nurses, medical assistants, behavioral health specialists and others are co-located in new team spaces where they can more easily collaborate in real time on patient care. Fallon members will benefit from this model by receiving care that addresses their health care needs holistically.

Fallon’s Moving Forward Award recognizes a health care provider for excellence in one or more of the following three ways:

Reliant Medical Group is one of the largest medical groups in Central Massachusetts. It offers a full range of primary and specialty care services, including same-day medical care through its network of ReadyMED centers. Reliant has recently opened eight brand new state-
of-the-art medical offices in Worcester, Westborough, Leominster, Milford, Shrewsbury and Auburn, which are designed to facilitate the care team clinical model and provide patients with a variety of amenities.
As part of the Moving Forward Award, a $1,000 donation from Fallon on Reliant Medical Group’s behalf will be made to St. John’s Food for the Poor program – a nonprofit charitable organization that serves hot and nutritious meals and provides support programs in Worcester.
