Personal Wellness ProfileTM kit

The Personal Wellness Profile is a tool that’s available to employers who offer FCHP insurance. When your employees fill out a profile, we can help you design a health and wellness program for your company. Once your employees fill out the profile, we’ll provide individualized reports to them and detailed, yet private, reports to you (if more than 50 or more employees complete the profile).
