FCHP offers you three options: FCHP Direct Care (our plan with a limited provider network), FCHP Select Care and FCHP Flex Care Select. No matter which FCHP plan you choose, you’ll get:

- Up to $500 reimbursement ($250 for a individual contract) for fitness center memberships, town and school sports leagues and other healthy activities

Deductibles and common costs
Our plans have a deductible, which is a dollar amount that you have to pay before FCHP starts to pay for certain covered services. For example, if your plan has a $1,000 deductible, you'll pay the first $1,000 for certain services, and then FCHP will begin to pay for those services.

Peace of Mind Program™
The Peace of Mind Program is a referral program which allows FCHP members to receive certain types of specialty care at one of the following affiliated medical centers in Boston:
- Beth Israel Deaconess Hospital
- Brigham and Women’s Hospital
- Children’s Hospital*
- Dana-Farber Cancer Institute
- Massachusetts General Hospital
- Tufts Medical Center*
How to get services through the Peace of Mind Program
- You must first see an FCHP in-plan specialist for your medical condition.
- Within three months of seeing the FCHP in-plan specialist, you may request a Peace of Mind Program preauthorization from your PCP to one of the Peace of Mind Program facilities.
- The request must be for the same type of specialist and for the same medical condition in step 1 (above).
- Your referring physician sends in a request for preauthorization to FCHP for approval.
- Your referring physician should provide assistance to you in selecting a specific Peace of Mind Program provider. If your referring physician is unfamiliar with Peace of Mind Program options, he or she may direct the member to call our Care Services Department for assistance at
1-508-799-2100 or 1-800-333-2535, ext. 69138. - You will receive a letter from FCHP notifying you if the Peace of Mind Program preauthorization has been approved.

* FCHP Direct Care members have access to Children's Hospital and Tufts Medical Center through the Peace of Mind Program. Children's Hospital and Tufts Medical Center are contracted FCHP Select Care hospitals and are available to FCHP Select Care members at any time.
