More coordinated care


NaviCare HMO SNP and NaviCare SCO provide coordinated medical, prescription and support services for people who are 65 or older, live in our service area, are eligible for MassHealth Standard and Medicare or MassHealth Standard alone. NaviCare provides all services covered by Medicare and MassHealth Standard, plus other services that your loved one may need.

With NaviCare, your loved one's primary care physician will work with a team, called the Primary Care Team, to develop a care plan with you and your loved one.

Your loved one will have a Geriatric Support Service Coordinator who will help arrange for community services. Your loved one will also have a Nurse Case Manager and Navigator who will help manage health care needs along with doctors, social workers and other health care specialists.

Summit ElderCare®

For people 55 and older who qualify for nursing home care, Summit ElderCare is a complete medical care, insurance and social support program designed to keep your loved one at home, not in a nursing home.

Our team of social workers, nutritionists, rehabilitation specialists, home health care specialists, and doctors, nurses and other professionals who specialize in geriatric care oversee your loved one’s personalized care in all settings, including at home and at our PACE adult day health centers. Our services include full coverage for prescription drugs and medical transportation, geriatric case management, adult day services, resources for caregivers, and much more.*

* In some cases, a monthly premium may be required based on the participant's income or assets. All services must be provided or authorized by the PACE interdisciplinary team, except emergency services. PACE participants may be held liable for costs for unauthorized or out-of-PACE program agreement services.

Fallon Health is an HMO/HMO-POS plan with a Medicare contract and a contract with the Massachusetts Medicaid program. NaviCare is a voluntary program in association with MassHealth/EOHHS and CMS. Enrollment in Fallon Senior Plan and NaviCare depends on contract renewal.
