More coordinated care
NaviCare provides coordinated medical, prescription and support services plus all services covered by Medicare and MassHealth Standard, plus other services that your loved one may need.
With NaviCare, your loved one's primary care physician will work with a team, called the Primary Care Team, to develop a care plan with you and your loved one.
Your loved one will also have a Geriatric Support Service Coordinator who will help arrange for community services and a Nurse Case Manager and Navigator who will help manage health care needs along with doctors and other specialists.

People who are 65 or older, live in our service area, and are eligible for MassHealth Standard and Medicare or MassHealth Standard alone

Complete coordinated care
Summit ElderCare is a complete medical care, insurance and social support program designed to keep your loved one at home, not in a nursing home. Our team works with you to address your loved one's individual needs.
Our team of social workers, nutritionists, rehabilitation specialists, home health care specialists, and doctors, nurses and other professionals who specialize in geriatric care oversee your loved one’s personalized care in all settings, including at home and at our PACE adult day health centers.
Our services include full coverage for prescription drugs and medical transportation, geriatric case management, adult day services, resources for caregivers, and much more.*
