Medicare group plans
FCHP offers high-quality health plans for Medicare-eligible employees and their spouses. Our staff will work with you one-on-one to develop a plan design that best fits your groups' needs.
Plan, benefit and network choices
Employers may choose to offer Fallon Senior Plan Premier (HMO), Fallon Senior Plan Premier Preferred (PPO) or both plan options, if the group meets size requirements. Both plans provide comprehensive health and wellness benefits with a tailored network of providers. We also offer Fallon Companion Care, a plan for retirees that live outside of our Medicare Advantage service area.
Our PPO plan offers access to more providers than our HMO plan and includes in-network and out-of-network benefits. For both the PPO and HMO plans, members must live in the Fallon Senior Plan service area.
