How to join Summit ElderCare

We want to make sure that Summit ElderCare is the right fit for you. That’s why we do a thorough assessment of your needs, at no cost to you. Our enrollment nurse coordinators guide you and your caregivers through the assessment and enrollment process.

Who is eligible to join Summit ElderCare

To be eligible for the program, you must meet these guidelines:

  • Be age 55 or older
  • Meet the Social Security Title XVI disability standards—if between the ages of 55 and 64
  • Need enough assistance to qualify for a nursing home level of care, but wish to remain at home
  • Be able to live safely in the community, according to the Care Team
  • Live in our service area
  • Hampden and Worcester counties
  • In Essex County: Andover, Lawrence, Methuen and North Andover
  • In Hampshire County: Easthampton, Granby, South Hadley and Southampton
  • In Middlesex County: Acton, Ashby, Ayer, Bedford, Billerica, Boxborough, Carlisle, Chelmsford, Concord, Dracut, Dunstable, Groton, Hudson, Littleton, Lowell, Marlborough, Pepperell, Shirley, Tewksbury, Townsend, Tyngsborough and Westford

How much does Summit ElderCare cost?

The amount you pay for Summit ElderCare depends on your income and assets. You may qualify for assistance from Medicare or Medicaid. Or you may pay the complete cost privately.

Many participants have no monthly premiums, and there are no additional out-of-pocket costs once you join.

As a Summit ElderCare participant, you receive all your health insurance—including Medicare Part D prescription coverage—and all your health services through the program. There are no copayments, no deductibles, no coinsurance and no paperwork for approved services, including hospitalizations, medical supplies and much more.
