Vision care provider
Routine eye exams
A routine eye exam is an examination where an eye doctor checks your vision and your eye health. To find a routine eye care provider, choose "Optometry" or "Vision Center" in the "Specialist" drop-down menu below.
Specialty vision care
As an FCHP member, you’re also covered for specialty eye care to treat an eye condition or injury. Your plan may require a referral or preauthorization, so be sure to check with us before scheduling an appointment with a vision care specialist. To find a vision care specialist, choose "Ophthalmology" in the "Specialist" drop-down menu below.
