Fallon Community Health Plan to offer more options for seniors in 2010

FCHP announces new Senior Care Options

Worcester, Mass., October 26, 2009—As Medicare beneficiaries consider health care coverage for 2010, Fallon Community Health Plan (FCHP), a nationally-recognized, not-for-profit health care services organization, announced today a unique, new product for seniors that combines in one package all Medicare and Medicaid benefits, as well as additional benefits and services designed to promote independence and quality of life.

NaviCareSM HMO is available to those age 65 and over in Worcester County who are enrolled in MassHealth Standard. NaviCare is available at a $0 premium with $0 copayments for those who qualify. Members receive high quality care through the program’s ability to combine health and social services. All health care is coordinated by a team of doctors, specialists, nurses, and social workers who develop a plan of care specific to each member.

In addition to NaviCare, FCHP also will continue offering other products and programs for Medicare beneficiaries, including a variety of plan choices for its Medicare Advantage product, called Fallon Senior Plan™. Effective January 1, 2010, Medicare beneficiaries who choose Fallon Senior Plan can select coverage from eight health plan options, including one with a $0 monthly premium, and $0 copayments for routine annual physicals for those enrolled in Fallon Senior Plan HMO plans.

FCHP continues to sponsor Summit ElderCare®, a Program of All-Inclusive Care for the Elderly (PACE). With locations in Charlton, Worcester and Leominster, Summit ElderCare is an insurance, medical care and social support system all in one convenient package that includes home care, personal care and even prescription drugs, when approved.
