Provider Tools registration

It's easy to register for Provider Tools. Complete just one registration form to access all of them!

Step one:
Download the registration packet, which includes the provider enrollment form and Billing Agency Authorization form.

Step two:
Complete the requested information.

Step three:
Fax your registration form to 508-797-4292 or mail the registration information to:

Upon receipt, FCHP will verify your provider status and assign you a username and password. FCHP will contact you with your personal information. Then, just go to the Provider Tools section and begin using these valuable tools.
