Fallon Community Health Plan appoints Kelly A. Cummings to Vice President of Care Coordination

Worcester, Mass., May 29, 2009― Fallon Community Health Plan (FCHP), a nationally recognized, not-for-profit health care services organization, announces the appointment of Kelly A. Cummings to Vice President of Care Coordination. In this position, she will be responsible for the strategy and execution of FCHP’s clinical operations related to utilization management, case management, government services clinical programs, disease management and maintaining the health plan’s care coordination accreditation. Cummings previously held the position of Assistant Vice President of Care Coordination.

Cummings came to FCHP from Health New England, where she held myriad roles, including management of the health services and member support and relations departments.

Cummings received a bachelor’s degree in nursing from St. Joseph College in West Hartford, CT, and a master’s degree in health services administration from Sage Graduate School in Albany, NY.

Cummings is a member of the Wilbraham Community Association, Phi Kappa Phi and proudly supports Red Sox Nation.

She resides in Wilbraham, Mass., with her husband Brian.
