Eligibility

We are happy that you are considering joining Fallon Senior Plan. To join Fallon Senior Plan (except Fallon Senior Plan Value 1 and Fallon Senior Plan Value 2), you:

• Must be enrolled in Medicare Part A and B
• Must continue to pay your Medicare Part B premium if it’s not paid for by Medicaid or another third party
• Must live in the Fallon Senior Plan service area
