How can members find out which plan they have and what their copayments are for prescription drugs?*

They can find all this information on the FCHP membership ID card!

  • The name of the plan is at the top of the card, next to the FCHP logo.
  • The letter after "Rx" indicates whether or not the member has prescription coverage through FCHP.  "Y" means that the member does have coverage, and "N" means that he or she doesn't.
  • If the member has prescription drug coverage, the numbers after "COPAY" are the copayments. The first number is the copayment for Tier 1 drugs, the second number is the copayment for Tier 2 drugs, and the third number is the copayment for Tier 3 drugs.

How do I use the drug formulary?

A formulary is a listing of drugs, classified by therapeutic category or disease class that are considered preferred therapy for a given population and are the drugs to be used by our providers in prescribing medications. The online drug formulary, which includes both brand-name drugs and generic drugs, lists prescription medications covered by Fallon Community Health Plan. Generic drugs have the same active-ingredient formulary as a brand name drug. Generic drugs usually cost less than brand name drugs and are rated by the Food and Drug Administration (FDA) to be as safe and effective as the brand name drugs.

Self-injectable medications used in the home setting are included in these formularies. Injections given by health care professionals in a health care setting are not. The Fallon Senior Plan formulary does not include durable medical equipment that is needed for use of medication.

First, you will need to select what formulary you would like to search. Select "Fallon Senior Plan™" if you would like to search for drugs in the Fallon Senior Plan formulary. Select "FCHP commercial plans" if you would like to search for drugs in any other plan formulary except FCHP MassHealth or our Commonwealth Care type 1 plan.
