Summit ElderCareSM
Summit ElderCare (SE)* is a Program of All-inclusive Care for the Elderly (PACE) that gives elders and their caregivers an innovative choice in health care. Participants can access most medical services at the SE center while enjoying the independence of living in their own homes, their own communities. SE delivers individualized quality care by one team of medical professionals who work together to address each participant's specific needs.

* Although Fallon Community Health Plan (FCHP) sponsors SE, a participant does not have to be an FCHP member to enroll.

Who is eligible to participate in Summit ElderCare?
Any person age 55 or older who:
- is able to live safely at home.
- lives in Worcester County, Marlboro or Hudson.
- is certified by the Massachusetts Division of Medical Assistance's screening agent as meeting Medicaid nursing facility clinical criteria.
- is accepted for enrollment by SE.
