As a user of My FCHP, it is very important to read these Terms and Conditions. This document describes the terms of use that rule My FCHP. For Fallon Community Health Plan policies concerning Web privacy please refer to the FCHP Web Privacy Policy. Please note that the information contained on this Web site may be updated as FCHP continues to expand the capabilities of My FCHP. Fallon Community Health Plan also has the right to make changes to the products, benefits and features described within the Web site.

Web site security, confidentiality and registration
Fallon Community Health Plan has implemented the best technical industry practices in order to provide the utmost security for our users’ online transactions. At Fallon Community Health Plan, security is our biggest priority, and we have taken the appropriate measures to ensure the confidentiality, integrity and availability of personal information about our members, providers, employers, brokers or anyone else that uses our Web site.
Fallon Community Health Plan has a strict, high-level of protection for transmitting secure personal health information between a user’s browser and My FCHP. All users of My FCHP are required to create a user ID and password to further protect our members’ privacy. Once you have registered with My FCHP, you will be mailed a PIN within three to five business days, giving you added protection against unauthorized access to your My FCHP account.
All information stored within the Fallon Community Health Plan Web site is kept in a secure server, with the protection of a firewall. All users of the Web site and/or My FCHP are required to have a browser that supports 128-bit encryption, which is needed to scramble transmitted information assisted by encryption technology. Once you have entered a secure section of the Web site and/or My FCHP, you can confirm the page you are viewing is secure by the padlock symbol located on the bottom-right of your Web browser.
With regard to My FCHP, all data is received as confidential, and FCHP will not disclose any identifiable data to third parties. FCHP defines identifiable data as the first and last name, mailing address, e-mail address(s) and telephone numbers. Members can give others permission to access such information by filling out a legal document.
It is very important not to share or give your My FCHP user ID, password, or Personal Identification Number (PIN) with anyone, and also give thought to the person(s) who may have access to your e-mail account.
The information you provide us within My FCHP is only used to help us with a response to your request. Because we want to ensure that your request is researched to its fullest, the information you provide within My FCHP may be shared with other Fallon Community Health Plan employees. As a member, it is important for you to know that all employees of Fallon Community Health Plan are required to complete privacy training and also sign confidentiality agreements to ensure that any private information they receive during their work is treated with the highest confidentiality and respect.
Communications with Fallon Community Health Plan
Fallon Community Health Plan will not respond to any specific requests made about your own personal medical or health questions or concerns. FCHP recommends that all personal medical or health questions or concerns are made to your primary care physician. If you require an immediate response to non-medical questions or problems related to your health care coverage, please call Fallon Community Health Plan Customer Service at 800-868-5200 (TDD/TTY: 877-608-7677), Monday through Friday, 8 a.m. to 6 p.m. E-mail that is sent to Fallon Community Health Plan may not be encrypted and may be accessed and viewed by other Internet users without your knowledge when it is electronically transmitted to Fallon Community Health Plan. Fallon Community Health Plan offers members the opportunity to reach a Member Services Representative who is available to discuss personal information related specifically to a member’s health care. The Member Services Representative will share your comments, concerns and questions with those Fallon Community Health Plan employees who are most apt to handle such requests.

At Fallon Community Health Plan, we’re always looking for ways to expand the features and functions of My FCHP to make your health care experience with us more efficient, accurate and accessible for all our users. The Web Privacy Policy will be updated to accurately reflect how information and features are being used as additional features, functions and information are added.
