Eligibility

To be eligible for one of our Medicare Advantage group plans, members must:

• Have access to a Medicare group plan through an employer or a union health and wellness organization
• Be enrolled in Medicare Part A and Part B
• Continue to pay their Medicare Part B premium if not otherwise paid for under Medicare or by a third party.
• Have permanent residence in the Fallon Senior Plan service area

Certain individuals who have end-stage renal disease (ESRD) may also be eligible for a Medicare group plan.
